Your primary care doc would be best to start with as having access to your current medical records and being able to relate your symptoms to any of your existing conditions, if any. Hypnic jerks are suggested by your description and if a specialist, such as a neurologist is needed, your doc will be able to coordinate locally.

You likely are describing paroxysmal limb movements of sleep (nocturnal myoclonus), which is a sleep disorder variant. The intensity and frequency can vary. Usually, not necessary to treat, but, this needs a full evaluation. An overnight sleep study will help objectify diagnosis.
