It varies. If fever is your marker it should be gone in a few days. If your marker is congestion/fullness it may be weeks or more. You can kill all the germs but the mechanical effect of sinus chamber swelling may keep sinuses full. The swelling that results from the infection must fade and the chambers reopen and drain normally before fulness is gone.

Most " sinus infections" are not bacterial and don't need antibiotics. Z packs are often over-used and do not treat sinus infections well. If you are not better after a round of 10 days of amoxicillin then the diagnosis is in question. In my experience, the most common telemedicine in the US is a visit is a 20 to 40-year-old asking for antibiotics for a sinus infection. See a doctor in person.
