Kids fail to gain weight when they take in less calories than they need. There are medical conditions where they loose calories by failing to absorb them. I suspect your feeding plan has not worked as you wanted. Infants get 80%/side with 5 min on each breast/90%at 10 min. If you feed constantly you may not be letting the breasts recover. Work this out with your doc.

Continue to breastfeed and discuss a supplemental formula with your physician. Drink fluids and your baby should be fed every three hours if more often by demand. An analysis of factors including GI malabsorption may be considered but usually low intake is a more common cause of poor weight gain.
