This is the HT public information site. We offer to answer medical questions to those who submit a description of a medical problem. We do not offer private services like notes, treatments, prescriptions, etc. There s a subscription/fee based concierge HT site with some of those services. If your doc has provided a summary you should have their office staff provide a summary & have the doc sign it

Doctor notes are issued on well documented physical exams with/ without other tests as needed that result in a diagnosis. Based on a working diagnosis, such notes can be used for work related concerns/excuses. Your best reference remains your doc, who can further clarify how work excuses work in general.
