"Urinary bladder is poorly distended" means bladder is not distended enough. I am not sure why the radiologist used this phrase. Usually, mild or moderate or severely distended is used but not poorly distended unless the bladder should be distended during the procedure, e.g. VCUG test, but there is not enough volume. "Prostate is upper limits in size" means the prostate size is still normal.
