Dx w/ na fatty liver in 2014. in last 3 years my alt has gone from 59 to 18 and ast went from 23 to 19 still within normal limits but ratio is now greater than 1. recent us showed "echogenic liver". do i need to worry about fibrosis/cirrhosis?

All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, the ratio is irrelevant when the enzyme levels are normal, as is in your case. Wish you good health!
