Cold Sores are typically caused by Herpes Simplex Virus 1. At infection, the virus enters lip cells and causes a localized necrosis (cell death) resulting in a sore. As the sore clears, the HSV 1 virus becomes dormant in any nerve cells it has infected but not killed. Stress & illness can cause more cold sores. The immune system is actively trying to fight off the infection- enlarged lymph node.
