Not effective at all; it might slightly reduce pain and itching, but nothing more. Acyclovir ointment is better, but even that isn't very effective for most people. The best treatment for an oral herpes outbreak (cold sore) is a single large dose of valacyclovir (2 grams) orally immediately when the sore starts.
