Docosanol (Abreva) doesn't work at all against oral herpes (cold sores). Acyclovir (Zovirax) ointment is better, but still not highly effective for most persons. The best treatment by far is a single large oral dose of valacyclovir (2 grams) taken immediately after start of an outbreak.
