Possible. HIV, hepatitis B, and CMV (Cytomegalovirus) have been well known to be transmitted via breast milk.

Infection is related to the number of viral or bacterial particles (the "dose") so a single exposure would be less likely than multiple exposures over time. Breast milk is a bodily fluid that can transmit HIV, hepatitis and other infections, and you will want to limit the baby's exposure. This would be "universal precautions" territory.
