I am from south india. my father(age - 78) has trouble urinating. did ultrasound abdomen scan and the doctor told there is a problem with his prostate gland(cystitis, mild prostatomegaly, complex cystic lesion in prostate). is big surgery required?

Not necessarily. He should consult a urologist. Medications may be sufficient to resolve the matter. If he needs surgery, it could be done through the urethra without opening up the abdomen.
