In some cases, a tfcc tear in combination with ulnar positive variance can be difficult to treat. A joint leveling procedure with an ulnar shortening osteotomy can be a complicated surgery. This is best done by a competent hand surgeon and the recovery can be prolonged. It is often necessary to undergo a second surgery later to remove the plate used to secure the ulnar bone shortening.
