With a history of GERD, swallowing difficulties should be evaluated with endoscopy. Get a referral to a GI specialist for testing.

There are many other structural and functional causes that will have to be explored, cancer is always part of the differential diagnosis, not as likely as others. Best would be to have your condition looked at early, ideally by your primary care doc who can further refer to a gastroenterologist.
