I had a rectal prolapse fixed in september 2020. today even with no bowel movement the toilet has been full of bright red blood and blood clots. what does this mean?

It would be unusual for bleeding to arise from the surgical site done in "September 2020" , if this were the first time occurrence of same since the operation. Heavy vaginal bleeding can occur at age "45", after pre-menopausal delays in ovulatory function. See your prolapse surgeon for an exam and guidance.
