Mri and xrays are not showing any fractures nor swelling. what can be causing my hand pain after a fall four years ago when i landed on the fleshy part of my hand underneath my thumb?

You probably have thenar eminence pain. You can google for this article: How to Diagnose, Treat, and Prevent Thenar Eminence Pain I doubt if the current pain has anything to do with the fall years ago unless this pain has persisted in the last 4 years.

OA or osteoarthritis of the CMC joint or basilar joint of the thumb metacarpa, near the trapeziuml is a common source of pain there. Sometimes trauma can set up a cycle of irritation or synovisits, exacerbating symptoms but NOT the cause ot the osteoarthritis. It can also bee what we call true true and unrelated, you had trauma, you have OA and they just seem to correlate by time and location.
