Work up needs to include scopes with small and large bowel biopsies, repeat stool studies, trial of bile acid sequestrant among others. Review of current meds has to be done as well, Whipple disease among others will have to be ruled out as well. A gastroenterologist specializing in malabsorptive syndromes at a large tertiary center should be involved at this point.
