Chronic diarrhea 10+ yrs, it's getting worse i was 190lbs 3 yrs ago, now i'm 150lbs! hospitalized twice in the last 2 months for dehydration & wt loss. all stool, blood tests normal, stool osmolality gap +36, ct ok, imodium not working, help?

It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. You have obviously been examined by doctors and it is not possible to do better in this forum. Drink enough water daily so your urine is mostly colorless. You may wish to see a GI specialist. Work up needs to include scopes with small and large bowel biopsies, repeat stool studies, trial of bile acid sequestrant among others. Review of current meds has to be done as well, Whipple disease among others will have to be ruled out as well. A gastroenterologist specializing in malabsorptive syndromes at a large tertiary center should be involved at this point.
