Do you mean that this is being taken sub lingual. I would stop this and see a doctor to get examined.

Yes, your symptoms are suggesting an allergy type of reaction, to nicotinamide or any other chronic meds you may be taking, commonly used blood pressure meds for example. Being examined timely is critical in order to have your condition, angioedema, diagnosed and treated properly. A local urgent care or ER would be best in that regard.
