A hygienist removes tartar and plaque from around and in between teeth to make sure all surfaces of teeth are smooth and clean to help prevent gum disease and cavities. If the fillings are faulty, they need to be replaced. New set of xrays and a check up would clarify and answer your questions.

Hygienist's instruments cannot "carve out" restorations. Not possible. If a filling is loose, it may be dislodged which is a good thing... the filling needed replacement. If the hygienist removed calculus, then food can be retained in those spaces. It appears you may have bone loss/periodontitis. You'll need to further explore your condition with your dentist.
