Sesamoiditis getting much worse. tried epat, ice, boot, test, heat. now hurts to even sit with my boot on. swollen and red wont go down. is it likely i will need surgery?

Surgery : Are you sure you have the correct diagnosis? Have you considered or try a custom orthotics made by a podiatrist? I recently wrote about this problem. If all else fails then surgery is indicated. Alternative to removing the sesamoid bone was to cut the bone in half in a transverse plane . I would also recommend a custom orthotics made by a podiatrist
