Any ideas why a healthy 48yo male would have a thumbnail-sized red patch on tip of tongue for about 2 months? the only symptoms - burning feeling in the mouth and smooth papilae at the tip. candida test came back negative and no sores are present?

See derm/oralsurg: I would suggest that you be evaluated by either a dermatologist or oral surgeon. This could be related to trauma or might be something more significant. Seek derm or oral surgeon care and closely monitor the lesion for any increase in size , color, or texture. Depending on these changes, a bx or further testing might be a possibility.
