Merits urology exam: This merits a urology consultation. The lump could be a blood clot in a vein or a growth warranting removal. The slow urinary stream can be assessed by flowmetry which quantifies flow and the amount of urine remaining in the bladder can be imaged using an ultrasound machine. Finally, a digital rectal exam can assess the prostate for evidence of cancer.

Scrotal lump: In a 39 year old a common finding is a sebaceous cyst if it is in the skin. However, without additional information, this is not a certainty. The slow urine flow is not related to the lump. In a young man it could be due to prostate infection, urethral narrowing from a stricture or other cause that requires examination. Prostate cancer is not likely at age 39. See a urologist for evaluation.
