Hsv2 positive 5+ years, never had outbreak but have constant vulvar pain, intense burning, occasional itching and localized right hip pain. could this be outbreaks without lesions/ulcers?

Possibly: We know that people without symptoms or eruptions can still shed & transmit herpes virus which lies in nerve roots. I assume herpes virus in nerve roots in lower vertebrae could cause neuropathic pain in your case. A neurologist would be best to answer your question, however.
