Arrhythmia: An EKG (or heart monitor) detects problems with the rhythm of the heart. You should click the button and document any time you feel any chest pain no matter where it is, heart palpitations (racing heart), skipped heart beats, shortness of breath or lightheadedness

Any symptom : Any unexplained symptoms you should log in and specifically the symptoms you presented with in order to have the test done, palpitations or irregular heart beats for example. Your symptoms will be temporally correlated with your heart rhythm.
