Hard to say: The description is vague. Some of these are self healing viruses or just fluid trapped inside the eardrum. Sometimes they are pus/germ filled infections that might takes weeks to resolve if not treated. If/when it bothers the friend enough they should have it evaluated.

See doctor: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. It would be prudent to see your doctor. Four days in pain is too long to wait. It is imperative to ascertain if the person has middle ear infection that could cause lasting damage if not treated promptly.
