Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. If heating pad and paracetamol have not resolved the issue it would be prudent to see your doctor. Wish you and your son good health!

Needs to be examined: Please contact his pediatrician or GP. Above the right hip the appendix, if inflamed can cause pain among others and that has to be ruled out or in as it is not going to respond to conservative measures. Alternatively consider an urgent care facility, in case his doc unavailable.
