I am due my annual medicare wellness visit. i want to focus a lot on advance care planning. in general, about how much time (15 minutes ?) will i have since internist very busy. medicare does not list a time limit.

As much as needed: For medical purposes 2 documents are important: Health Care Proxy and MOLST or Medical Orders for Life Sustaining Therapies. You will make best use of doctor's time if you study and/or complete documents ahead of time and save pertinent questions for the actual discussion. There is no specific time limit as billing can be done on time, doc will be limited by his or her schedule.
