May be infection: I would recommend that you see your healthcare provider. Virus or allergies are common causes of sinus symptoms which could be treated with otc meds and/ or sinus irrigation. It is recommended to avoid antibiotics if you have had less than 10 day of symptoms. However severe sinus symptoms for this long (a month) could be associated with bacterial infection or less likely fungal infection.
