My friend was prescribed piroxicam (20mg twice daily) for 5 days. on 4th day,she got severe stomach pain with black stools,egd revealed duodenal ulcers. how come such a short course of nsaid caused ulcers? and why was she asymptomatic till 3rd day?

Feldene: Piroxicam (Feldene) can lead to esophagitis, gastritis, glossitis, hematemesis, melena, rectal bleeding in 0.1 to 1% of people. Per Drugs.Com the recommended dose for osteoarthritis is 20 mg orally once a day. CYP450 2C9 poor metabolizers may have higher levels. Sometimes doctors use higher "off label" dosing of medications.
