I have an enlarged lymph node for years. it shrinks after antibiotics then after treatment it stays shrunk for a while then becomes enlarged again. is it likely it is reactive and not malignant?

Enlarged lymph node: You didn't say where the lymph node was. I would anticipate that a lymph node enlarged due to cancer would not intermittently shrink down. Still, it would be good to have your doctor check it out at your next visit.

Watch and wait: You may feel the node every couple of weeks, if the lymph gland doubles in size, becomes painful, ulcerates, or other nodes become enlarged, or you develop fever, or lose weight, you should see a doctor. Wish you good health!
