Heat Stroke.: Heat stroke occurs with exercise and usually present with progressive disorientation. There certainly may be muscle symptoms involved including pain and cramping. Malignant hyperthermia occurs with exposure to inhalational anesthesia and succinycholine.

Death: It does not happen. Mh can be thought of as a super allergic reaction. Calcium channels are activated in muscles. Super severe muscle damage from extreme exercise (rhabdomyolysis) could i guess potentially trigger it. I would not worry about it. If you've ever worked out so hard you threw up take that and then keep exercising for another couple hours it might get dangerous. I wouldn't worry.
