Operculum: The small tag of tissue that forms on the distal aspect of an erupting wisdom tooth is what you are referring to. You most likely lack space for that tooth. The time to visit is now, to determine if that tooth should be extracted.

Pain: Go now. if the wisdom tooth is erupting normally your dentist may remove the operculum to make you more comfortable, or s/he may refer you for treatment. If your wisdom tooth is not erupting normally, your GP will probably refer you for treatment. The point is, don't wait. Seek professional help now.
