Need oral treatment: No ointment or cream is effective. The anti-herpes drugs valacyclovir and acyclovir by mouth are entirely safe in pregnancy. However, no treatment helps much unless started within 2-3 days after onset of a cold sore. But you could plan on having a supply of drug on hand in case of another outbreak. Treatment is very simple: a single dose of valacyclovir 2 g within 1-2 days of onset works wonders.
