Enlarged cervical lymph nodes +-2 years. one side more prominent. multiple ultrasounds and fna reveal them to be reactive and no significant change in size over time. no other apparent infections. antibiotics not effective. is removal necessary?

NO: however, a lymph node biopsy will give a definitive diagnosis. Ultrasounds will not define tissue pathology v. inflammation or reactive lymphadenopathy.
