Toe: Although you have used Ketoconazole, toe fungus would be the most likely cause. Along with anti fungal make sure you keep foot dry as possible. Use dryer if necessary. If problem persists likely need to be seen. An online picture with a visit is often sufficient to resolve an issue like this.
