Something to ask: the practitioner who has diagnosed it and mentioned surgery. If it is not giving you any symptoms, these things often heal up without problems. But if you sought help for a symptom such as hearing loss, dizziness, ringing, and if the perf is large, then the symptoms could remain or get worse. Only someone looking could tell you how likely it is to heal on its own.
