Depends : That is a fairly large perforation. It may heal on its own within 4 to 6 weeks. However, it is quite possible that it will not fully heal. At that point, you need to consider surgical repair. It's not mandatory, if you don't plan on swimming or getting the ear wet. However, most people would opt to have it repaired.
