Hard to know: The origin of this issue can range from simple speech delay to autism. A thorough evaluation by a developmental pediatrician & speech/language pathologist is needed. In the US the public school system nationwide has a program called Child Find which seeks to find & begin treatment of any such child. Contact both his doc & the school system involved to start the process.
