Go to the ER: It sounds like you have epididymitis and need antibiotics. You don't mention how long you've had symptoms but testicular torsion which must be treated within about 6 hours to save the testicle and testicular tumor which would develop over weeks or months would both be included in the differential (possible) diagnosis. A testicular ultrasound will confirm the diagnosis. See a doctor NOW.
