Is it possible to have multiple benign cysts in liver, and spleen and this be diagnosed as liver cirrhosis without scans?

not really: It's not possible to even know you have benign, or any cysts in your liver without some kind of scan (ultrasound, CT, or MRI) or direct visualization by a surgeon. However, the diagnosis of liver cirrhosis can be made with a thorough history, complete physical exam, and laboratory studies without needing a scan at all but it would help to confirm the diagnosis and rule out other causes of symptoms
