Another culture: The patogen may be not sensitive to the antibiotic , may need another culture from infection site.

Gum infection: May be the wrong antibiotic or the wrong type of treatment. It sounds as if you should be under the care of a Periodontist, a gum/bone specialist, to resolve your chronic infection issue.
