May be systemic: Oral bacteria are very often the cause of bad breath, but not all the time. Some people's breath is affected by their chemistry, which can be detected in their breath the same way alcohol on someone's breath is not from their mouth - it's transferred from the bloodstream to the lungs, and expelled out the airway. Do your best with brushing and flossing, and try a mouthwash like smartmouth first.
