I have panic disorder and my blood pressure dropped from 120/79 to 110/65. can blood pressure spikes of 180/124 from panic attacks damage the heart and cause this change? should i be concerned?

Probably not.: Unless blood pressure is very, very high, it does not tend to pose an immediate risk. High blood pressure otherwise tends to pose a cardiovascular risk when it is sustained over at least some weeks or months. Temporary increases, like yours, tend to be a part of normal life, in temporary excitement or exercise. So you don't need to panic about this!
