Medical evaluation: The pattern and clinical symptoms are consistent with a disc herniation in the Lumbar Spine. I would see a Doctor as soon as possible to assess neurological function, and ideally get an mri of the Lumbar Spine. The severity and weakness makes this more urgent, before assuming to proceed on a conservative treatment course such as PT/Chiro, NSAIA, steroids, epidural steroid injections.

Get lift to Doctor: Sorry you are hurting, but once the pain reaches moderate to severe, it often takes strong, injectable options to get ahead of it. Also want to be sure it is what you think, and not something else. Should get to Urgent Care or ER for diagnosis and relief, then manage at home.
