Sometimes: It is fairly easy to tell if there is a massive tear in the rotator cuff by physical exam. What is more difficult is determining if there are small tears as simple bursitis and tendonitis can look the same. Thankfully the treatment for small tears is usually non surgical in most people. Mri can help with the diagnosis.

See ORS: I would suggest seeing an orthopedic surgeon in your area who specializes in shoulders or who has done an arthroscopy/ sports medicine fellowship. He/She can examine you thoroughly and give you a precise diagnosis. Other diagnoses in your age group( shoulder instability, SLAP tears, other labral tears, AC jt.pain,referred pain from neck, etc.) need to be ruled out. Best of Luck!
