MRI first: Based on 2 months of symptoms, I would suggest an MRI to look for a problem that x-ray and physical exam can not detect. Cartilage, ligament and tendon problems can be better detected with an MRI, and these can be common sources of pain and swelling in the ankle. I would not suggest a cast without a definitive diagnosis. If I can help, then join my care team at www.healthtap.com/dr-clarkeholmes
