You should: take him to see his doctor or to the emergency room today. There are many possibilities of what this could be, some more urgent than others , such as a torsion (twisting) of his testicle or one of the structures nearby, a hernia, a strain in the groin muscles, or other injury. The only way to tell is to have him examined.
