What is advance care planning?

Your health care: Advance care planning is a way for your family and your doctor to know what is important to you, what matters to you, what makes life meaningful for you and what your personal preferences towards end of life care and medical treatment really are. Writing and sharing this plan will give everyone peace of mind if you were very unwell.

Advance care plan: Advance care planning is an opportunity to understand what the future might holds and to say what treatment someone would and would not want. It helps people, their families and their healthcare teams plan for future and end of life care. This makes it much easier for families and healthcare providers to know what the person would want - particularly if they can no longer speak for themselves.
