National website: There is a national website where you can find stories, complete and download a copy of a plan and even use the free learning interactive learning sessions. Plans are written in a range of languages. The link is: http://www.advancecareplanning.org.nz. Your GP practice can give you a plan or help you to complete a plan. Talk to your GP or a healthcare worker who is involved in caring for you.
