If a patient has hx of sever psychiatric side effects from steroid shot, would you prescribe oral steroid for ear fluid after infection treated with 2 rounds of antibiotics or allow fluid to resorb? is 30 mg of prednisone alot?

Hard to say.: Wasn't there when steroid shot was given, & I don't know what the "psychiatric side effects" were (or their severity). Not having examined the pt, I don't know how much fluid's collecting in the ear, or other opts. (No response to antibiotics x2 rounds may be due to a host of reasons; problem may still be infectious). One thing's certain: similar psych side effects possible; report it right away.

Steroid: I would use with caution. Fluid behind ear not life threatening like an allergic reaction or asthma attacknrequiring steroids. Discuss your past issues with steroids with your doctor. Maybe a nasal steroid spray instead....
