Acute DVT Rx: npatient treatment (with or without pulmonary embolism): 1 mg/kg/dose every 12 hours or 1.5 mg/kg once daily. Outpatient treatment (without pulmonary embolism): 1 mg/kg/dose every 12 hours. Obesity: Use actual body weight to calculate dose; dose capping not recommended; use of twice daily dosing preferred
